An Individual Pupil Provision Map to monitor interventions for SEND pupils. Interventions and assessments can be edited to personalise for individual school needs. The key feature of this IPM is that it is an ‘at a glance’ snapshot of the provision that a pupil is receiving for each term of an academic year. It is a very useful monitoring tool and can form part of a SEND child’s paperwork to share with parents, carers and external professionals. In our school, this provision map works alongside a Support Plan and a One Page Profile (see other listings) to form a child’s One Plan paperwork.

This has been a key feature in getting some of our applications approved for assessment. The table has been completed with examples for information only.

A simple, single page template to make note of team discussions during PPA sessions. In our school, our LSAs join teachers for part of their weekly PPA time and I have found this a useful tool to monitor how this time is being used and the impact it has on planning and preparation to meet pupils’ needs. Effective dialogue between teachers and LSAs during PPA time allows for joint working and clear understanding between all professionals working with children within the classroom. Using this template also means that any staff who are absent from discussions can receive a copy of the notes so that they too have an understanding of what is happening in the week ahead.

A form that SENCOs/Inclusion Managers can use in school for teachers to complete and return, with supporting evidence, to raise concerns about potential SEND in pupils. This is aligned with the SEND Code of Practice which states that “Every school is required to identify and address the SEN of the pupils that they support…” (6.2). It also highlights key data groups so that this information can be used to help identify provision and support.
